This survey supports Measure 4 on the 2026 P4P Scorecard and will help structure the Optimization Group breakout session during the Fall Semiannual Meeting. \nSurvey data will be used to pre-match each hospital with three peer hospitals for short\, structured conversations focused on shared challenges\, practical strategies\, and mutual support.
