Each site can access its respective scorecard by logging into the Performance Reports webpage. \nThe midyear scorecard provides a snapshot of your site’s current progress\, but it is not your final P4P score. Final scores will incorporate all 2025 data and will be shared in early 2026. \nNext Steps for Your Champion Team: \n\nReview Your Scorecard: Please carefully review your site’s scorecard to ensure all data and calculations are accurate.\nSubmit Contestations to OBI: If you have concerns or identify potential errors\, please use the 2025 Scorecard Contestation Form to communicate any issues to the OBI team. The contestation submission period will be open from August 15 through August 26.\n\nTo submit a contestation\, please access the OBI Contact Us form.
