Fall 2026 Optimization Group Pre-Survey Due

Please complete the Fall 2026: Optimization Group (OG) Pre-Survey by August 14, 2026. This survey supports Measure 4 on the 2026 P4P Scorecard and will help structure the Optimization Group breakout session during the Fall Semiannual Meeting.

Survey data will be used to pre-match each hospital with three peer hospitals for short, structured conversations focused on shared challenges, practical strategies, and mutual support. Results and hospital names will be shared during the session to support continued collaboration.
